We are seeking a APRA registered nurse for our Northam practice. The Practice Nurse will use their expertise to improve the health of Wheatbelt residents. This includes triaging of patients, performing ECG’s, spirometry and ear syringing, administering immunisations, assisting with surgical procedures, managing recalls and reminders, and working collaboratively with health practitioners to provide the best possible customer experience and care.

It's essential to have strong work ethic and committment to professional standards and ethics, be able to work collaboratively, and have a good undertanding of Chronic Disease, Health Assessments, Care Plans, and Team Care Arrangements.

We are seeking a Dietitian to join the WHN team to provide clinical services to clients in a primary care setting under the Integrated Chronic Disease Care (ICDC) program funded by WA Primary Health Alliance. ICDC is a chronic disease management outreach program for clients with diabetes, cardiovascular and respiratory disease. Participants can access a dietitian, diabetes educator, podiatrist, and physiotherapist for their chronic illness.

We are seeking a Diabetes Educator to join our team to provide clinical services to clients in a primary care setting under the Integrated Chronic Disease Care (ICDC) program and mixed billing. WHN seeks expressions of interest from Diabetes Educators for both an employee and contractor model.
