ICDC Program

The Integrated Chronic Disease Care (ICDC) program services the Wheatbelt areas.

What is the ICDC Program?

The ICDC program assists people with chronic disease to access allied health services and manage their disease.

How can the Care Coordinator assist you?

  • Help to plan your allied health provider appointments
  • Update your GP about your progress
  • Provide information and support about your chronic condition
  • Show you how to access other support services such as tele-health
  • Encourage and support self management

What do I need to do?

See your GP for a referral to the ICDC program and have a current GP Care Plan and Team Care Arrangement.

After hours medical assistance: In an emergency call 000 or present to your nearest Regional Hospital emergency department. If you have a non- emergency and would like to consult with a GP then call Telstra Health on 1800 225 523. The service is free to access for Australian residents who reside in the wheatbelt or who are temporarily residing in the Wheatbelt. This service can be accessed before 8am and after 6pm Monday-Friday, before 8am and after 12pm Saturday and all day Sunday and any Public Holidays.
