To ensure sound governance reflecting a culture of continuous quality improvement.

Collaborate with key stakeholders, influence local primary health care policy, planning and service delivery.

To ensure sound financial management, compliance with all legal and contractual requirements and mitigation of identified risks.

PREVENTION & EARLY INTERVENTION

Goal 1

Support General Practices to implement the current National Immunisation handbook, including the Australian Standard Vaccination Schedule, the National Immunisation Program and the GP Immunisation Incentives scheme.

Goal 2

Establish a community resilience group.

ACCESS

Goal

Facilitate access and support GP's to provide optimal care and contribute to the achievement of the best possible health outcomes for older people living in residential aged care facilities.

INTEGRATION

Goal

Working with relevant hospitals to improve local service planning, timely and appropriate exchange of patient health information and integration of care for patients, families and communities.

MANAGE CHRONIC DISEASE

Goal 1

Support General Practice to provide optimal care and contribute to the achievement of the best possible health outcomes for patients with diabetes.

Goal 2

Support GPs to provide early intervention and optimal care and contribute to the achievement of the best possible health outcomes for patients with mental health disorders, and assist in the reduction of the impact of mental disorder on individuals, families and communities, in collaboration with other mental health services as appropriate.

Goal 3

Support GPs to provide optimal Asthma and Diabetes care for patients and contribute to the best possible health outcomes for patients with Asthma.

GP SUPPORT

Goal

Support management of change in the primary care environment.

QUALITY SUPPORT

Goal

Support quality in General Practice.

CUSTOMER FOCUS

Goal

To improve the interface between consumers and GP.

WORKFORCE SUPPORT

Goal

To support attraction, recruitment and retention of a GP and their family and maintain an appropriate primary care workforce.
