Rural Palliative Care



This project begain in 2008 with an aim to increase patient access to quality palliative care services available in the Wheatbelt.  Achievements so far include:

  • Forming of an Expert Reference Group which includes GPs, Nurses, Aged Care Facility Managers and stakeholders to discuss issues realtign to Palliative Care services in the Wheatbelt.
  • Funding nurses in the Wheatbelt to attend continuing education events regarding Palliative Care therefore, improving the skills of nursing staff.
  • Production of the Patient Held Record for palliative patients to keep a diary of their health care to imporve communication between health care providers throughout the course of their treatment.
  • Participation in Carers Expos in Merredin, Moora and Northam to raise public awareness of services available in the region.
  • Production of the Wheatbelt Palliative Care Directory which can be accessed by both members of the public and health professionals.
